Stroke Risk Factors

Some risk factors can be managed or controlled through medication and lifestyle changes, while others, such as age and family history, cannot be changed. Understanding which risk factors apply to you is an important step toward reducing your overall risk. 

A Malaysian study found that more than 70% of ischaemic stroke patients had underlying hypertension, almost half had diabetes, and about one-third had high cholesterol at the time of their stroke. This shows just how closely stroke risk in Malaysia is tied to a small number of common, often manageable conditions.

Who Is at Risk of Stroke?

Stroke risk is rarely caused by a single factor. In most cases, it results from a combination of medical conditions, lifestyle habits, and factors such as age that cannot be changed. National Health and Morbidity Survey data shows that more than 60% of Malaysian adults have at least one cardiovascular risk factor, which means stroke risk affects a significant portion of the population, often without people being fully aware of it. 

 

H2: Medical Risk Factors 

1. High Blood Pressure

Ischaemic Stroke

High blood pressure, or hypertension, is the single most significant risk factor for stroke. Over time, consistently high blood pressure damages and weakens blood vessel walls throughout the body, including in the brain, making them more likely to narrow, block, or rupture.

According to NHMS 2023, hypertension affects approximately 29.2% of Malaysian adults, and a separate national study found the figure to be closer to 49% when accounting for undiagnosed cases. 

2. Diabetes

Diabetes significantly increases stroke risk by damaging blood vessels and accelerating the build-up of fatty deposits within them over time.  

Diabetes prevalence in Malaysia reached 15.6% in 2023, continuing a long-term upward trend from earlier surveys. A particularly concerning finding is that a large proportion of Malaysians with diabetes remain undiagnosed, especially among younger adults, meaning many people are living with this risk factor without knowing it.

3. Atrial Fibrillation

Atrial fibrillation, or AFib, is an irregular heart rhythm in which the heart's upper chambers beat out of sync with the lower chambers. This irregular rhythm allows blood to pool and form clots, which can then travel through the bloodstream to the brain and cause an ischaemic stroke. 

AFib is considered one of the most under-recognised and undertreated risk factors for stroke, partly because it can occur without obvious symptoms. Identifying and treating AFib with appropriate medication significantly reduces stroke risk in affected individuals.

4. High Cholesterol

High cholesterol contributes to the build-up of fatty deposits, known as plaque, inside the walls of arteries and increases the likelihood of a blood clot forming or blocking blood flow to the brain.  

Hypercholesterolaemia affects approximately one in three Malaysian adults, and like high blood pressure and diabetes, it often produces no noticeable symptoms until it contributes to a more serious event such as a stroke or heart attack.

5. Previous Stroker or TIA

Having had a previous stroke or transient ischaemic attack significantly increases the risk of a future stroke.  

This is why identifying and managing the underlying cause of a first stroke or TIA is such a critical part of long-term care, rather than something that ends once a patient is discharged from hospital.

 

Lifestyle Risk Factors

Beyond underlying medical conditions, several everyday habits and lifestyle factors meaningfully increase stroke risk over time. 

  • Smoking – Damages blood vessels, raises blood pressure, and makes blood more likely to clot, all of which directly increase stroke risk. 

  • Obesity – Places additional strain on the heart and blood vessels and is closely linked to the development of high blood pressure, diabetes, and high cholesterol. 

  • Physical Inactivity – Associated with a higher risk of stroke, as regular physical activity helps maintain healthy blood pressure, blood sugar, and cholesterol levels. 

  • Excessive Alcohol Consumption – Can raise blood pressure and contribute to irregular heart rhythms, both of which increase stroke risk. 

  • Unhealthy Diet – A diet high in salt, saturated fat, and processed foods, and low in fruits and vegetables, contributes meaningfully to the development of the medical risk factors described above, including high blood pressure, diabetes, and high cholesterol. 

 

Stroke Risk Factors in Younger Adults

Stroke is increasingly affecting younger adults in Malaysia, not just the elderly. Hospital admission data has shown a substantial rise in stroke incidence among people under 65, with the sharpest increases occurring among those in their late 30s. 

In younger adults, stroke is often linked to the same underlying risk factors seen in older patients, particularly undiagnosed high blood pressure and diabetes, but can also be associated with less common causes, such as: 

  • Inherited blood clotting disorders
  • Heart conditions present from birth
  • Recreational drug use

 

Frequently Asked Questions

Does having one risk factor mean I will definitely have a stroke? 

No. Having one or more risk factors increases your overall likelihood of stroke but does not make it certain. Many people with risk factors such as high blood pressure or diabetes never have a stroke, particularly when these conditions are well managed through medication, regular monitoring, and lifestyle changes. The goal of understanding your risk factors is to manage them proactively, not to view a stroke as inevitable. 

At what age does stroke risk start increasing? 

Stroke risk begins increasing notably after the age of 55 and continues to rise with each subsequent decade. However, this does not mean younger adults are not at risk. Rising rates of high blood pressure, diabetes, and obesity in younger populations in Malaysia mean stroke is increasingly being seen in people in their 30s and 40s as well. 

Can stress cause a stroke? 

Chronic, ongoing stress is associated with several behaviours and physiological changes that can increase stroke risk indirectly, including elevated blood pressure, poor sleep, unhealthy eating habits, and increased likelihood of smoking or excessive alcohol use. While stress alone is not typically classified as a direct cause of stroke in the same way as hypertension or diabetes, managing stress is considered a meaningful part of overall cardiovascular health.
