MEMBERSHIP TERMS AND CONDITIONS – Cherish Card

Terms and Conditions

Definitions-

• Sunway Medical Centre Penang Sdn Bhd, a company registered in Malaysia under company registration number 199201006107 (237611-X).
• SMCP Cherish e-Card is issued by Sunway Medical Centre Penang.
• Card Holder refers to any individual holding a valid SMCP Cherish e-Card.

1. The SMCP Cherish e-Card is a privilege card which gives Card Holder the benefits of enjoying discounts, privileges, and events from time to time offered by Sunway Medical Centre Penang.
2. The SMCP Cherish e-Card is non-transferable and non-assignable and is only for the exclusive use of the Card Holder only.
3. The SMCP Cherish e-Card is NOT a point accumulation card. It does not allow or require Card Holder to collect points for gift exchanges or similar purposes.
4. The SMCP Cherish e-Card is NOT a credit card, debit card or the like.
5. Discount privileges are valid only upon presentation of your SMCP Cherish e-Card during registration. No reimbursement or refund will be provided for previous or back-dated transactions if the Card Holder fails to present the SMCP Cherish e-Card during registration.
6. Discount privileges apply strictly to self-pay cases only.
7. Unless otherwise stated, the card cannot be used together with any other cards, promotions, or senior citizen discounts offered by the hospital.
8. SMCP reserves the right to discontinue, amend or add any terms and conditions, benefits or privileges granted by this card as deemed necessary without prior notice.
9. SMCP shall not be liable for any loss or damage arising from any unauthorized use of the SMCP Cherish e-Card.
10. If the e-Card is lost, inaccessible, or compromised, Card Holders may request a replacement e-Card via SMCP, subject to verification.
11. SMCP reserves the right to suspend or terminate the Card Holder’s privileges if any terms, conditions, or benefits are misused or abused.
12. SMCP undertakes that all personal information supplied for e-card registration shall be handled in accordance with applicable laws and will not be disclosed to third parties without the prior written consent of the Card Holder unless required by law.
13. The SMCP Cherish e-Card benefit and privileges are redeemable after a period of seven (7) working days from the date of successful registration.
14. The SMCP Cherish e-Card is a lifetime membership programme with no renewal required.
15. The SMCP Cherish e-Card is available to Malaysians and expatriates aged 60 years and above.
16. The discounts offered for Sunway Home Healthcare are only valid on non-packages services. It is also not valid on medical supply and mileage charges.
