Mammogram or Ultrasound: Which Breast Screening Do You Need? When a breast screening appointment is on the calendar, one question often comes up first: mammogram or ultrasound? The two tests are frequently mentioned together, but they are not interchangeable. Knowing the difference can help you understand what your doctor recommends and why.   What Is a Mammogram? A mammogram uses low dose X-rays to look for changes in the breast, including changes that may be too small to feel. 1. Best Suited For Breast cancer screening, particularly as women get older Detecting suspicious changes before symptoms appear Further assessment when recommended by your doctor 2. Penang Records Among the Highest Stroke Cases in Malaysia: What the Data Tells Us Stroke is one of the most time-critical medical emergencies a person can face. In Malaysia, it is already the third leading cause of death and the numbers are not going down. For people living in Penang, the picture is particularly concerning. State-level data places Penang among the highest in the country for recorded stroke cases, making awareness, early recognition, and access to rapid treatment more important than ever.   Stroke Is Malaysia's Third Leading Cause of Death According to a 2022 study published in Cerebrovascular Diseases Extra, there were 47,911 incident stroke cases, 19,928 deaths, and 443,995 prevalent cases of stroke in Malaysia in 2019 alone. That translates to more than 130 new stroke cases every single day across the country. Beyond the mortality figures, stroke is [...]

How Advanced Technology Is Transforming Scoliosis Surgery in Malaysia For many patients and families, a scoliosis diagnosis comes with a wave of fear. This is especially true when surgery is being considered. Fear of nerve damage. Fear of long scars. Fear of a lengthy, painful recovery. Fear of whether the surgery will even work. These concerns are legitimate. Scoliosis surgery is complex. The spine houses the spinal cord and major nerve roots, and the margin for error in implant placement is narrow. At Sunway Medical Centre Penang, a new generation of intraoperative technology is directly addressing that challenge and changing what is possible for patients in the northern region.   The Problem With Traditional Spine Surgery In conventional spine surgery, a surgeon works from pre-operative imaging, which includes X-rays and scans taken hours or days before the procedure [...]

Haze in Penang | Health Risks, API Explained and How to Stay Safe

Kategori: blog

Haze in Penang: What the Air Is Doing to Your Body and How to Protect Yourself Penang is once again experiencing poor air quality. After a brief improvement in early October, haze conditions returned on 8 October 2026, with three of the state's four monitoring stations recording unhealthy Air Pollutant Index (API) readings. According to the hourly readings on the Department of Environment's APIMS portal, Seberang Perai peaked at API 156, Minden at 156 and Seberang Jaya at 154, while Balik Pulau stayed at a moderate level throughout the day. Penang State Executive Councillor for Youth, Sports and Health Daniel Gooi advised the public to limit outdoor activities and wear face masks when heading out. Haze episodes in Malaysia tend to be recurring rather than one-off events, particularly during the Southwest Monsoon season, which runs from May to September. Even as the [...]

Stroke Emergency? Call Us Immediately
Our stroke team is on standby 24 hours a day, every day
04-373 9111

When Should Stroke Rehabilitation Start?

The rehabilitation process begins within the first 24 hours of a patient being medically stabilised following acute stroke treatment.

In the early stages, rehabilitation may involve gentle positioning, passive movement of affected limbs, and supported sitting or standing under the supervision of our physiotherapy team.

These initial steps are important for preventing complications such as:

  • Muscle stiffness
  • Joint contractures
  • Pressure injuries
  • Deep vein thrombosis

Our Neuro Rehabilitation Services

Stroke rehabilitation is delivered through a team-based approach involving physiotherapy, occupational therapy, and speech-language therapy, depending on the nature and severity of the patient's deficits.

At Sunway Medical Centre Penang, all three disciplines are available as part of our Neuro Rehabilitation Therapy Package, which is recommended for patients recovering from:

  • Stroke
  • Traumatic brain injury
  • Spinal cord injury
  • Brain tumour
  • Other neurological conditions

1. Physiotherapy

Physiotherapy after stroke focuses on restoring movement, strength, balance, and physical function. Our physiotherapists work with stroke patients on mobility training to help them regain the ability to sit, stand, walk, and move safely and independently.

  • Strength and endurance exercises — To rebuild muscle function in affected limbs.
  • Balance and coordination training — To reduce the risk of falls, which are a significant concern for stroke survivors.
  • Early mobilisation — Supported movement that begins as soon as the patient is medically stable

2. Occupational Therapy

Occupational therapy after stroke focuses on helping patients relearn and regain the practical skills needed for daily life. A stroke can affect a person's ability to perform tasks they previously did without thinking, for example, dressing, eating, writing, bathing, and preparing food.

Specific interventions our occupational therapists provide include:

  • Fine Motor Training — Focused rehabilitation for hand and upper limb function, helping patients regain dexterity and grip strength for everyday tasks.
  • Neurological Rehabilitation — Structured programmes to enhance independence and function for patients with neurological impairments caused by stroke.
  • Activities of Daily Living (ADL) Training — Helping patients relearn essential daily tasks such as dressing, eating, grooming, and bathing through repeated, guided practice.
  • Splinting — Custom-made splints to support joint alignment, prevent contractures, and improve functionality during recovery.
  • Ergonomics and Body Mechanics Consultation — Advising patients on safe posture and movement patterns to protect the body and prevent strain during recovery activities.
  • Return-to-Work Consultation — For patients approaching discharge, helping them plan and prepare for a safe and supported transition back to their professional lives where this is a realistic goal.

3. Speech and Language Therapy

At Sunway Medical Centre Penang, our speech-language therapists assess and treat patients that experiencing:

  • Aphasia — difficulty producing or understanding spoken or written language
  • Dysarthria — slurred or unclear speech caused by weakness in the muscles used for speaking
  • Dysphagia — difficulty in swallowing, which can lead to aspiration pneumonia

For patients with speech and language difficulties, our therapists use structured exercises to retrain the muscles involved in speaking, improve word retrieval and sentence formation, and rebuild the ability to understand and express language in both spoken and written form.

Our Rehabilitation Equipment

Our Rehabilitation Centre is equipped with specialist equipment to support stroke rehabilitation across a range of functional needs and recovery stages.

Music Glove

An FDA-listed hand rehabilitation device that combines therapeutic movement with an interactive musical game. Patients complete hundreds of hand exercises per session in an engaging, motivating format, helping rebuild fine motor control after stroke.

FitMi Home

A full-body neurorehabilitation system that supports movement recovery across the hands, arms, core, and legs. FitMi Home guides patients through therapeutic exercises using wireless sensor pucks, with difficulty levels adapting automatically as the patient progresses.

FitMi HOME system

Tilt Table

The tilt table is used in the early stages of rehabilitation for patients who are not yet able to stand independently. This stimulates circulation, helps maintain bone density, reduces the risk of pressure injuries, and begins to prepare the neuromuscular system for standing and walking.

Hoist

The hoist is a mechanical lifting device used to safely transfer patients who have significant physical limitations and require assistance moving between positions. For example, from bed to chair, or from chair to standing. It supports both patient safety and the physical well-being of nursing and therapy staff during transfers.

Rise and Go Walker

The Rise and Go Walker is a specialist mobility aid designed to assist patients in the early stages of walking rehabilitation. It provides support during the transition from sitting to standing and during assisted walking.

HUR Exercise Machine

The HUR Exercise Machine is a pneumatically driven resistance training system designed specifically for rehabilitation and elderly patients. Unlike conventional weight machines, HUR equipment uses compressed air rather than weights, which allows for very precise, low-resistance starting loads that can be increased gradually as the patient gains strength.

Frequently Asked Questions

What is the difference between physiotherapy and occupational therapy?

Physiotherapy focuses on restoring physical function — movement, strength, balance, and the ability to walk and move safely. Occupational therapy focuses on helping patients relearn the practical skills needed for daily life, such as dressing, eating, writing, and bathing. In stroke rehabilitation, both work together as part of the same recovery programme. A physiotherapist helps the patient move again; an occupational therapist helps the patient live again.

Will I or my loved one ever walk or speak properly again?

Many stroke survivors do regain the ability to walk and speak, though the extent of recovery varies from person to person depending on the severity of the stroke, which part of the brain was affected, and how consistently rehabilitation is carried out. Some patients recover fully. Others recover significantly but may have some lasting effects. The brain has a remarkable ability to form new pathways through a process called neuroplasticity, and this is what rehabilitation works to stimulate. Setting realistic goals with your rehabilitation team and committing to consistent therapy gives every patient the best possible chance of meaningful recovery.

How many rehabilitation sessions will a stroke patient need?

There is no fixed number as it depends on the severity of the stroke, the specific deficits being addressed, and the rate of each patient's progress. Our rehabilitation team sets personalised short and long-term goals for every patient and reviews progress regularly, adjusting the programme accordingly. Some patients benefit from intensive daily sessions in the early weeks after a stroke, while others transition to less frequent outpatient sessions as they regain independence. The focus is always on what each individual patient needs to achieve their recovery goals.

Can I prevent another stroke from happening?

Yes. Identifying and managing the underlying cause of the first stroke is a critical part of reducing the risk of a second one. This includes controlling conditions such as high blood pressure, diabetes, high cholesterol, and atrial fibrillation through medication and lifestyle changes. For a full guide on what you can do to reduce your stroke risk, visit our Stroke Prevention page.

What to Expect During Stroke Rehabilitation

At Sunway Medical Centre Penang, our rehabilitation team is with your family every step of the way — from the first days in hospital through to discharge planning and beyond. Recovery takes time, patience, and consistency, but with the right support, meaningful improvement is possible at every stage.

Stroke Rehabilitation in Penang Stroke rehabilitation is the process of helping a patient relearn skills that were lost or affected when the stroke damaged part of the brain. How to Prevent a Stroke?

Making changes to our daily habits is one of the most effective ways to prevent stroke.

1. Blood Pressure Control

Controlling high blood pressure is the most impactful step we can take to reduce the stroke risk. High blood pressure is the leading risk factor for stroke, and keeping it within a healthy range (below 130/80 mmHg) significantly reduces chances of a stroke occurring.

This involves of:

  • Dietary changes
  • Regular physical activity
  • Quitting Smoking
  • Limiting alcohol intake
  • Managing Weight

2. Regular Physical Activity

Regular exercise can help strengthen the heart, improve circulation, maintain a healthy weight, and directly lower blood pressure, blood sugar, and cholesterol levels. Adults are recommended to exercise at least 150 minutes of moderate-intensity aerobic activity per week.

These activities include:

  • Brisk walking
  • Swimming
  • Cycling

3. Dietary Changes

Reducing salt intake is particularly important for managing blood pressure, and reducing saturated fat helps control cholesterol levels. Therefore, we must cut down on a diet that is high in:

  • Sodium
  • Saturated fat
  • Processed foods

Instead, practice a healthier diet that is rich in:

  • Fruits
  • Vegetables
  • Whole grains
  • Lean protein.

4. Quitting Smoking

Smoking is a direct and significant cause of stroke. It damages our blood vessels, raises blood pressure, reduces oxygen in the blood, and makes clotting more likely. Quitting smoking at any age reduces stroke risk, and the benefit begins within weeks of stopping. Smokers who quit can significantly reduce their stroke risk within a few years of cessation.

5. Limiting Alcohol

Excessive alcohol consumption raises blood pressure and increases the risk of atrial fibrillation, both of which are stroke risk factors. Reducing alcohol intake or stopping entirely is recommended, especially for individuals who already have other risk factors of stroke.

6. Weight Management

Excess body weight contributes to high blood pressure, diabetes, high cholesterol, and sleep apnea, all of which increase stroke risk. Maintaining a healthy weight through a combination of diet and regular physical activity is one of the most beneficial things a person can do for their long-term cardiovascular health.

7. Get Regular Health Screening

Many of the stroke risk factors like high blood pressure, high cholesterol, and diabetes have no symptoms. Identifying and managing these conditions through regular health screening is one of the most effective steps we can do to prevent and reduce stroke risk. Health screening should ideally begin by age 30 for all adults, and earlier if there is a family history of heart disease, stroke, diabetes, or hypertension.

At Sunway Medical Centre Penang, health screening packages are available to help you understand your current risk profile and take meaningful action on what you find.

Prevention of Recurrent Stroke

For stroke survivors, a second stroke is often more severe than the first, and it carries a higher risk of serious disability and death. Therefore, preventing a recurrent stroke is one of the most important priorities for long-term care.

Secondary prevention builds on all the measures above but places additional emphasis on identifying and directly treating the specific cause of the first stroke.

Some stroke survivors discontinue their preventive medications within months of discharge. Continuing prescribed medications and attending regular follow-up appointments are not optional but essential, even when you feel completely recovered.

Frequently Asked Questions

Is it too late to prevent a stroke if I already have high blood pressure or diabetes?

It is never too late. These conditions do increase stroke risk, but managing them effectively—through medication, lifestyle changes, and regular monitoring—significantly reduces the likelihood of a stroke, even when they have been present for years. Starting at any point is better than not starting at all, and consistent management matters far more than having perfect health to begin with.

How soon after a stroke should prevention measures begin?

As soon as possible. It is ideally before discharge from the hospital. The days and weeks immediately following a stroke or TIA carry the highest risk of a recurrent event, making early action critical. Your treating team will typically begin a prevention plan as part of your discharge discussion, including medication review and lifestyle guidance.

What part of the brain is most affected by stroke?

This varies from patient to patient, as a stroke can occur in any part of the brain depending on which blood vessel is affected. However, strokes affecting the brain's large hemispheres, particularly areas controlling movement and speech, are commonly seen because these regions are supplied by major arteries that are more frequently involved in both clot-related and bleeding-related strokes.

Can I stop taking my stroke prevention medication once I feel better?

No, and this is one of the most important points on this page. Feeling well does not mean your risk has gone away — it often means your medication is working. Stopping antihypertensives, statins, or antiplatelet medications allows the underlying conditions to return to levels that significantly increase stroke risk. If you are unsure about your medications, speak to your doctor before making any changes.

What is the difference between primary and secondary stroke prevention?

Primary prevention is about reducing the risk of a first stroke in someone who has not had one. Secondary prevention focuses on preventing a second stroke in someone who has already had a stroke or TIA. Both use the same tools — lifestyle changes and medication — but secondary prevention is usually more targeted and more intensive, based on identifying the specific cause of the first stroke.

Taking Action Is the Most Important Step

Understanding what puts you at risk of stroke is genuinely useful. But the part that makes a difference is doing something about it consistently and with the right support. Whether you are taking steps to protect yourself for the first time or managing your risk after a stroke, the path forward is the same: know your numbers, take your medications, make the lifestyle changes that matter, and stay connected with your healthcare team.

How to Prevent Stroke

Kategori: page

Prevention matters at every stage. Stroke Prevention Although stroke can cause significant disability, it is preventable. Highly effective stroke prevention often does not require any drugs or complex medical procedures. Instead, the steps are our everyday lifestyle habits and routine health management choices that are accessible to everyone.  For people who never had a stroke, the goal is to manage risk factors before one happens. For people who have already experienced stroke or TIA, risk of suffering a recurrence stroke is estimated 30% by 5 years, which make secondary prevention extremely important.  Stroke Emergency? Ischaemic Stroke Treatment

1. Thrombolysis: Clot-Dissolving Medication

Thrombolysis is one of the primary treatments for ischaemic stroke. It involves administering a clot-dissolving medication that breaks down the blood clot blocking blood flow to the brain, restoring circulation and limiting the extent of permanent brain damage.

Thrombolysis must be given within 4.5 hours of stroke onset to be effective. The earlier it is administered within that window, the greater the benefit.

2. Mechanical Thrombectomy: Stroke Clot Removal

Mechanical thrombectomy is a minimally invasive endovascular procedure that removes clots from brain vessels.

Based on current international guidelines, mechanical thrombectomy can be performed within 6 hours of symptom onset as standard, and up to 16 to 24 hours after onset in selected patients.

Stroke treatment at Sunway Medical Centre Penang follows the Ministry of Health Malaysia Clinical Practice Guidelines for Management of Ischaemic Stroke, ensuring every patient receives evidence-based care aligned with national standards.

Haemorrhagic Stroke Treatment

Treatment for haemorrhagic stroke focuses on controlling and stabilising the bleed through careful blood pressure management and medication to support clotting.

In cases where the bleed is significant or causing dangerous levels of pressure on the brain, surgical intervention by our neurosurgery team may be required to relieve that pressure or address the source of the bleed directly.

Medical Management During Stroke Treatment

Alongside acute interventions, all stroke patients receive ongoing medical management throughout their admission. Anti-platelet therapy such as aspirin is typically started within 24 to 48 hours of an ischaemic stroke once imaging confirms no bleeding is present. These medications reduce the tendency of blood to clot and lower the risk of a second stroke.

For patients whose stroke was caused by atrial fibrillation, an irregular heart rhythm that allows blood to pool and form clots that travel to the brain, anticoagulant medications are prescribed. Blood pressure, blood sugar, and body temperature are all monitored and managed carefully, as elevated levels of any of these during the acute phase are known to worsen stroke outcomes.

What Happens After Stroke Treatment?

Following acute treatment, stroke patients are admitted to our Intensive Care Unit (ICU) or High Dependency Unit (HDU) for close monitoring during the first 24 to 48 hours. Our stroke unit hospital team carries out continuous neurological checks, vital sign monitoring, swallowing assessment, and active management of any complications that arise including brain swelling, pneumonia, or deep vein thrombosis.

Once the patient is stable, they are transferred to our dedicated stroke unit where the recovery phase begins.

Rehabilitation is started as early as possible, often within the first 24 hours of stabilisation. Our physiotherapists, occupational therapists, and speech language therapists work with each patient based on the specific effects of their stroke and their individual recovery goals.

Recognised with the World Stroke Organisation (WSO) Gold Award

Sunway Medical Centre Penang has been recognised by the World Stroke Organisation (WSO) with the Gold Award for five consecutive quarters, a testament to our consistent standards in stroke prevention, acute treatment, and rehabilitation care.

The WSO Angels Awards recognise hospitals that demonstrate measurable, sustained improvements in stroke care delivery. Achieving Gold status across five consecutive quarters reflects our team's ongoing commitment to rapid response, evidence-based treatment protocols, and patient outcomes.

Sunway Medical Centre Penang is one of a select number of hospitals in Penang, Malaysia and Southeast Asia to maintain this level of recognition continuously. This reflects our ongoing commitment to upholding the highest standards in neurology, neurosurgery, and stroke care for patients across the region.

Stroke Treatment in Penang

At Sunway Medical Centre Penang, our stroke team is available around the clock to assess and begin treatment the moment a patient arrives. In stroke care, time is brain.

The right treatment depends on the type of stroke, its severity, and how much time has passed since symptoms began. Our team of neurologists, neurosurgeon, and interventional radiologist work together to determine the most appropriate treatment pathway for each patient without delay.

Frequently Asked Questions

Will stroke treatment fully restore the patient to how they were before?

This depends on the severity of the stroke, which part of the brain was affected, and how quickly treatment was received. Some patients recover fully, while others may experience lasting effects such as weakness, speech difficulties, or changes in memory and cognition. Recovery is not always linear — many patients continue to improve for months or even years after the stroke with consistent rehabilitation and support.

How long does stroke treatment take?

Thrombolysis is delivered as a drip over approximately one hour. Mechanical thrombectomy typically takes between one and two hours depending on the complexity of the case. These are followed by a period of close monitoring in the ICU or HDU for 24 to 48 hours, after which the patient is transferred to the stroke unit where rehabilitation begins. The overall hospital stay varies depending on stroke severity and the patient's rate of recovery.

Can a stroke happen again after treatment?

Yes, stroke survivors have a higher risk of experiencing a second stroke compared to the general population. This is why identifying the underlying cause of the first stroke — whether it is high blood pressure, atrial fibrillation, high cholesterol, or diabetes — is a critical part of the treatment process. Appropriate medication, lifestyle changes, and regular follow-up care significantly reduce this risk.

Can stroke treatment be done if the patient is on blood thinners?

Being on blood thinners does not automatically exclude a patient from stroke treatment, but it is one of the factors our stroke team assesses carefully before deciding on the most appropriate approach. Certain blood-thinning medications and their dosages can affect eligibility for thrombolysis. Our team will review the patient's complete medication history alongside imaging results and clinical assessment to determine the safest and most effective treatment option.

Is thrombolysis safe and are there side effects?

Thrombolysis is a well-established treatment that has been used in stroke care for decades and is recommended by international clinical guidelines for eligible patients. Like all medical treatments, it carries a small risk of side effects, the most significant being bleeding, including in rare cases bleeding in the brain. Our stroke team carefully assesses each patient's eligibility before administering thrombolysis to ensure the benefits outweigh the risks for that individual. For eligible patients, the potential benefit of limiting permanent brain damage far outweighs the risk.

At Sunway Medical Centre Penang, stroke treatment does not end when the acute procedure is complete. Recovery involves close monitoring, ongoing medical management, and structured rehabilitation tailored to each patient's specific needs and recovery goals. Our multidisciplinary stroke team supports patients through every stage of that journey, from the Emergency Department through to discharge and beyond.

How Is Stroke Treated

Kategori: page

Stroke treatment must begin as quickly as possible. Stroke Treatment at Sunway Medical Centre Penang Stroke is treated differently depending on whether it is caused by a blood clot blocking an artery (ischaemic stroke) or by bleeding in the brain (haemorrhagic stroke). These two types of strokes require completely different approaches. Ischaemic stroke accounts for approximately 76% of all stroke cases in Malaysia. Treatment focuses on restoring blood flow to the brain as rapidly as possible; while haemorrhagic stroke treatment focuses on controlling the bleeding and protecting surrounding brain tissue. Who Is at Risk of Stroke?

Stroke risk is rarely caused by a single factor. In most cases, it results from a combination of medical conditions, lifestyle habits, and factors such as age that cannot be changed. National Health and Morbidity Survey data shows that more than 60% of Malaysian adults have at least one cardiovascular risk factor, which means stroke risk affects a significant portion of the population, often without people being fully aware of it.

Medical Risk Factors

1. High Blood Pressure

High blood pressure, or hypertension, is the single most significant risk factor for stroke. Over time, consistently high blood pressure damages and weakens blood vessel walls throughout the body, including in the brain, making them more likely to narrow, block, or rupture.

According to NHMS 2023, hypertension affects approximately 29.2% of Malaysian adults, and a separate national study found the figure to be closer to 49% when accounting for undiagnosed cases.

2. Diabetes

Hand holding a blood glucose meter measuring blood sugar

Diabetes significantly increases stroke risk by damaging blood vessels and accelerating the build-up of fatty deposits within them over time.

Diabetes prevalence in Malaysia reached 15.6% in 2023, continuing a long-term upward trend from earlier surveys. A particularly concerning finding is that a large proportion of Malaysians with diabetes remain undiagnosed, especially among younger adults, meaning many people are living with this risk factor without knowing it.

3. Atrial Fibrillation

Young man has heart attack

Atrial fibrillation, or AFib, is an irregular heart rhythm in which the heart's upper chambers beat out of sync with the lower chambers. This irregular rhythm allows blood to pool and form clots, which can then travel through the bloodstream to the brain and cause an ischaemic stroke.

AFib is considered one of the most under-recognised and undertreated risk factors for stroke, partly because it can occur without obvious symptoms. Identifying and treating AFib with appropriate medication significantly reduces stroke risk in affected individuals.

4. High Cholesterol

Top view of unhealthy meal

High cholesterol contributes to the build-up of fatty deposits, known as plaque, inside the walls of arteries and increases the likelihood of a blood clot forming or blocking blood flow to the brain.

Hypercholesterolaemia affects approximately one in three Malaysian adults, and like high blood pressure and diabetes, it often produces no noticeable symptoms until it contributes to a more serious event such as a stroke or heart attack.

5. Previous Stroker or TIA

Having had a previous stroke or transient ischaemic attack significantly increases the risk of a future stroke.

This is why identifying and managing the underlying cause of a first stroke or TIA is such a critical part of long-term care, rather than something that ends once a patient is discharged from hospital.

Lifestyle Risk Factors

Beyond underlying medical conditions, several everyday habits and lifestyle factors meaningfully increase stroke risk over time.

  • Smoking – Damages blood vessels, raises blood pressure, and makes blood more likely to clot, all of which directly increase stroke risk.

  • Obesity – Places additional strain on the heart and blood vessels and is closely linked to the development of high blood pressure, diabetes, and high cholesterol.

  • Physical Inactivity – Associated with a higher risk of stroke, as regular physical activity helps maintain healthy blood pressure, blood sugar, and cholesterol levels.

  • Excessive Alcohol Consumption – Can raise blood pressure and contribute to irregular heart rhythms, both of which increase stroke risk.

  • Unhealthy Diet – A diet high in salt, saturated fat, and processed foods, and low in fruits and vegetables, contributes meaningfully to the development of the medical risk factors described above, including high blood pressure, diabetes, and high cholesterol.

Stroke Risk Factors in Younger Adults

Stroke is increasingly affecting younger adults in Malaysia, not just the elderly. Hospital admission data has shown a substantial rise in stroke incidence among people under 65, with the sharpest increases occurring among those in their late 30s.

In younger adults, stroke is often linked to the same underlying risk factors seen in older patients, particularly undiagnosed high blood pressure and diabetes, but can also be associated with less common causes, such as:

  • Inherited blood clotting disorders
  • Heart conditions present from birth
  • Recreational drug use

Frequently Asked Questions

Does having one risk factor mean I will definitely have a stroke?

No. Having one or more risk factors increases your overall likelihood of stroke but does not make it certain. Many people with risk factors such as high blood pressure or diabetes never have a stroke, particularly when these conditions are well managed through medication, regular monitoring, and lifestyle changes. The goal of understanding your risk factors is to manage them proactively, not to view a stroke as inevitable.

At what age does stroke risk start increasing?

Stroke risk begins increasing notably after the age of 55 and continues to rise with each subsequent decade. However, this does not mean younger adults are not at risk. Rising rates of high blood pressure, diabetes, and obesity in younger populations in Malaysia mean stroke is increasingly being seen in people in their 30s and 40s as well.

Can stress cause a stroke?

Chronic, ongoing stress is associated with several behaviours and physiological changes that can increase stroke risk indirectly, including elevated blood pressure, poor sleep, unhealthy eating habits, and increased likelihood of smoking or excessive alcohol use. While stress alone is not typically classified as a direct cause of stroke in the same way as hypertension or diabetes, managing stress is considered a meaningful part of overall cardiovascular health.

Knowing Your Risk Is the First Step

Understanding your personal risk factors is the foundation for taking action to reduce them. Many of the most significant risk factors for stroke, including high blood pressure, diabetes, and high cholesterol, can be identified through simple health screening and effectively managed once known.

Stroke Risk Factors

Kategori: page

Stroke Risk Factors Some risk factors can be managed or controlled through medication and lifestyle changes, while others, such as age and family history, cannot be changed. Understanding which risk factors apply to you is an important step toward reducing your overall risk.  A Malaysian study found that more than 70% of ischaemic stroke patients had underlying hypertension, almost half had diabetes, and about one-third had high cholesterol at the time of their stroke. This shows just how closely stroke risk in Malaysia is tied to a small number of common, often manageable conditions.  Stroke Emergency? How to Recognise a Stroke: BEFAST

BEFAST is a simple checklist that helps you recognise the most common signs of stroke quickly, in yourself or in someone else.

stroke symptom - lose balance

B
Balance

stroke symptom - blurred vision

E
Eyes

stroke symptom - one side of face is dropping or uneven

F
Face

stroke symptom - numbness in the arm or one side of the body

A
Arm

stroke symptom - difficulty in speaking

S
Speech

stroke symptom - call ambulance and come to hospital immediately

T
Time

  • B - Balance

Is the person suddenly having trouble with balance or coordination? A sudden inability to stand or walk steadily, or a feeling of dizziness that comes on without warning, can be a sign of stroke.

  • E – Eyes

Has the person suddenly lost vision in one or both eyes, or developed blurred or double vision? Sudden visual changes, especially without pain, should never be dismissed.

  • F – Face

Ask the person to smile. Does one side of the face droop or feel numb? A crooked or uneven smile is one of the clearest and most recognisable signs of stroke.

  • A – Arm

Ask the person to raise both arms. Does one arm drift downward, or is one side noticeably weaker than the other? Sudden one-sided weakness, even if mild, is a warning sign that should not be ignored.

  • S – Speech

Ask the person to repeat a simple sentence. Is their speech slurred, strange, or difficult to understand? Sudden difficulty speaking or understanding speech is a key indicator of stroke.

  • T – Time

If you notice any of these signs, even if they seem to pass quickly, call for emergency help immediately. Note the exact time the symptoms started, as this information is critical for the treating medical team.

Important Notes:

One of the most dangerous mistakes is waiting to see if symptoms go away before seeking help. If symptoms resolve on their own within minutes to hours, this may have been a transient ischaemic attack, or TIA, often called a mini stroke. A TIA is a serious warning sign that a full stroke may follow soon after, and it requires the same urgent medical attention as an actual stroke.

Do not drive yourself or the affected person to hospital. Call for an ambulance so that trained medical personnel can begin assessment and care immediately, even before arrival at hospital.

Other Warning Signs to Watch For

While BEFAST covers the most common and recognisable signs, stroke can present in other ways as well. Watch for any of the following appearing suddenly:

  • A severe headache with no known cause, particularly if it is the worst headache the person has ever experienced.
  • Sudden confusion or difficulty understanding what is being said.
  • Sudden numbness in the face, arm, or leg, especially when it affects only one side of the body.
  • Sudden trouble walking, along with dizziness or loss of coordination.

If any of these symptoms appear suddenly, treat the situation as a medical emergency and call for help immediately, even if the person does not show every BEFAST sign.

Frequently Asked Questions

Are stroke symptoms different in women than in men?

Men and women often experience similar core stroke symptoms, such as face drooping, arm weakness, and speech difficulty. However, women may sometimes experience subtler signs such as sudden fatigue, confusion, or general weakness rather than the more classic symptoms. Because these signs can be mistaken for something less serious, they are sometimes missed or dismissed, which can delay treatment.

Can stroke symptoms come and go?

Yes. If symptoms appear and then resolve on their own within minutes to hours, this may indicate a transient ischaemic attack, or TIA. A TIA should never be ignored, even if the person seems to recover completely, as it is a strong warning sign that a full stroke could follow.

What should I do while waiting for the ambulance to arrive?

Stay with the person and keep them calm and still. Note the exact time their symptoms first appeared, as this is critical information for the treating medical team. Do not give them food, drink, or medication. If they lose consciousness, place them on their side to keep their airway clear, and continue monitoring their breathing until help arrives.

Is a sudden severe headache always a sign of stroke?

Not always, but a sudden, severe headache unlike any the person has experienced before, especially if it occurs alongside other BE FAST symptoms, should be treated seriously. This type of headache can be a warning sign of haemorrhagic stroke, where bleeding occurs in the brain, and requires immediate medical assessment.

Know the Signs, Act Without Hesitation

Recognising stroke symptoms is only the first step. Acting on them immediately, without waiting or second guessing, is what gives someone the best chance at recovery. Understanding what puts a person at higher risk of stroke can also help you stay alert to the warning signs in yourself and your loved ones.

Stroke Symptoms and Warning Signs

Kategori: page

Stroke Symptoms and Warning Signs Stroke symptoms appear suddenly, often without warning. Recognising them in yourself or someone else, and acting immediately, can mean the difference between a full recovery and permanent disability.  In Malaysia, six new cases of stroke occur every hour, with an estimated 52,000 Malaysians affected each year. Despite this, awareness of stroke warning signs in Malaysia remains low, which delays treatment at the exact moment when speed matters most.  If you suspect a stroke, do not wait. Our Diagnostic Facilities

Sunway Medical Centre Penang is equipped with the full range of technology required to diagnose stroke rapidly and accurately. Our on-time facilities include a 24-hour:

  • CT scanner
  • MRI Brain and MRA Brain
  • Hybrid Catheterisation Laboratory (cerebral angiography and endovascular procedures, echocardiography, and full cardiac monitoring)

What Tests Are Done When You Have a Stroke?

Diagnosing a stroke involves several tests carried out in quick succession. Brain imaging is the most critical, but blood tests, heart monitoring, and vascular assessment all play important roles in building a complete picture of the stroke and its cause.

1. CT Scan of the Brain

A CT scan is almost always the first test performed when a stroke is suspected. It is fast and highly effective at identifying one of the most urgent questions in stroke care: is there bleeding in the brain?

This matters enormously because the three main types of strokes:

  • Ischaemic stroke – caused by a blocked blood vessel
  • Haemorrhagic stroke – caused by bleeding

These three types of stokes require completely different treatments. Giving clot-dissolving medication to a patient who is actually bleeding in the brain would be dangerous. A CT scan provides this critical information within minutes.

Beyond ruling out bleeding, a CT scan can also show early signs of brain tissue damage from reduced blood flow, as well as brain swelling and other structural changes.

2. MRI Brain (Magnetic Resonance Imaging)

MRI (Magnetic Resonance Imaging)

An MRI uses powerful magnetic fields and radio waves to produce highly detailed images of the brain. Unlike a CT scan, it does not use radiation. It is significantly more sensitive to early stroke damage and can detect changes in brain tissue within minutes of a stroke occurring, even when a CT scan would not yet show anything abnormal.

When used with a technique called Diffusion-Weighted Imaging (DWI), MRI is considered the gold standard for confirming acute ischaemic stroke. DWI detects the precise area of brain tissue that has been deprived of blood and oxygen, making it especially valuable for identifying small strokes deep within the brain, and strokes in the brainstem or cerebellum, which are very difficult to see on a standard CT scan.

3. MRA Brain (Magnetic Resonance Angiography)

MRA Brain is a specialised type of MRI that focuses specifically on the blood vessels in and around the brain rather than the brain tissue itself. It produces detailed images of the cerebral blood vessels without the use of radiation or contrast dye, making it a safe and non-invasive option for examining vascular structure.

MRA is used to identify narrowed, blocked, or abnormal blood vessels, including aneurysms, which are bulges in a blood vessel wall that carry a risk of rupturing, and arteriovenous malformations (AVMs), which are abnormal tangles of blood vessels that can bleed into the brain. These conditions require specific management and cannot be reliably detected on a standard CT or MRI alone.

MRA is often performed alongside MRI Brain as part of a combined neurological assessment, giving the medical team a complete picture of both the brain tissue and the blood vessels supplying it.

4. Cerebral Angiography

For patients who may need a procedure such as mechanical thrombectomy to remove a clot, cerebral angiography may also be performed. This is a more advanced imaging technique in which a thin, flexible catheter is inserted into a blood vessel in the wrist or groin and guided through to the blood vessels of the brain under live X-ray guidance. A contrast dye is then injected to produce real-time, high-resolution images of blood flow inside the brain.

At Sunway Medical Centre Penang, cerebral angiography is performed by our Consultant Interventional Radiologist inside our Hybrid Catheterisation Laboratory (Cath Lab). This is a specialised facility equipped to both diagnose and treat stroke in the same room, removing any delay between confirming the diagnosis and beginning the intervention.

Supporting Tests That Identify the Cause

Knowing that a stroke has occurred is only part of the picture. Identifying why it happened is equally important, because the cause determines what medication or intervention is needed to prevent a second stroke.

1. Electrocardiogram (ECG)

Checks the heart's electrical rhythm. This is particularly important because an irregular heartbeat called atrial fibrillation (AFib) is one of the most common causes of ischaemic stroke. When the heart beats irregularly, blood can pool and form clots that travel through the bloodstream to the brain. Identifying AFib means the right medication can be started to prevent this from happening again.

2. Echocardiogram

An ultrasound of the heart may also be performed. This examines the structure of the heart chambers and valves and checks for blood clots or abnormalities that may have been the source of a clot that reached the brain.

2. Carotid Ultrasound

Uses sound waves to examine the carotid arteries in the neck, which are the main blood vessels carrying blood from the heart to the brain. Narrowing or plaque build-up in these arteries is a significant risk factor for stroke. Identifying this allows the team to plan treatment, whether through medication or a procedure, to reduce the risk of a future event.

Frequently Asked Questions

What is the difference between a CT scan and an MRI for stroke?

A CT scan is performed first in an emergency because it is fast and can immediately rule out bleeding in the brain, which determines whether clot-dissolving treatment is safe to give. An MRI is more detailed and more sensitive to early stroke damage, and is used after the acute situation is stabilised to confirm the diagnosis and assess the full extent of injury to the brain.

Can a blood test detect a stroke?

A blood test cannot confirm or diagnose a stroke on its own. However, blood tests are an essential part of the overall assessment because they help rule out other conditions that can mimic stroke symptoms, check whether certain treatments such as clot-dissolving medications are safe to administer, and identify underlying risk factors such as high cholesterol, diabetes, or clotting disorders.

Can a stroke be missed on a CT scan?

Yes, it is possible for a CT scan to appear normal in the very early stages of an ischaemic stroke, particularly for small strokes or strokes located in the brainstem or cerebellum. This is one of the reasons MRI is used as a follow-up when CT results are inconclusive, as MRI is significantly more sensitive and can detect stroke damage that a CT scan may not yet show.

How soon should I seek help after a stroke?

You should seek emergency help immediately; the moment symptoms appear. The most effective stroke treatments have strict time windows — clot-dissolving medication must be given within 4.5 hours of symptom onset, and outcomes from clot removal procedures are significantly better the sooner they are performed. Types of Strokes

1. Ischaemic Stroke

Ischaemic Stroke

Ischaemic stroke occurs when a blood vessel supplying the brain becomes blocked, usually by a blood clot. This blockage cuts off blood flow to the area of the brain beyond it, causing that tissue to be deprived of oxygen. Ischaemic stroke is by far the most common type, accounting for approximately 76 to 85% of all stroke cases in Malaysia.

Blood clots that cause ischaemic stroke can form in two main ways:

  • Clot forms directly in the brain (Thrombotic Stroke) — A clot may form within a narrowed artery in the brain itself, often due to a build-up of fatty deposits over time.
  • Clot travels from elsewhere in the body (Embolic Stroke) — A clot may form elsewhere, commonly in the heart, and travel through the bloodstream until it lodges in a smaller blood vessel in the brain.

2. Haemorrhagic Stroke

Haemorrhagic Stoke

Haemorrhagic stroke occurs when a blood vessel in or around the brain ruptures and bleeds. The bleeding both deprives the surrounding brain tissue of blood and causes pressure to build up inside the skull, which can damage brain cells further. Although haemorrhagic stroke is less common than ischaemic stroke, it tends to be more severe and carries a higher risk of death or significant disability.

Common causes of haemorrhagic stroke include:

  • High blood pressure — Long-standing, poorly controlled high blood pressure gradually weakens blood vessel walls over time, making them more prone to rupture.
  • Aneurysms — Weak, bulging points in a blood vessel wall that can rupture and bleed into the surrounding brain tissue.
  • Arteriovenous malformations (AVMs) — Abnormal tangles of blood vessels that are present from birth and can bleed unpredictably.

3. Transient Ischaemic Attack (TIA), or Mini Stroke

Mini Stroke - Transient Ischaemic Attack

A transient ischaemic attack, often called a mini stroke, occurs when blood flow to part of the brain is temporarily blocked. The symptoms are identical to those of a full stroke, but they resolve on their own, usually within minutes to a few hours, and do not cause lasting damage that can be seen on standard imaging.

Despite resolving on its own, a TIA should never be treated as a minor event. It is a serious warning sign that the same blood vessel problem that caused the TIA could cause a full, permanent stroke soon.

How Common Is Stroke in Malaysia?

Stroke is a significant and growing public health concern in Malaysia. Beyond ranking as the third leading cause of death nationally, data from Ministry of Health hospital admissions between 2008 and 2016 found a substantial increase in stroke incidence among people under the age of 65, with the sharpest rise occurring in the 35 to 39 year age group, increasing by 53.3% in men and 50.4% in women over that period.

This trend reflects a broader pattern across Malaysia and the wider Southeast Asian region, where rising rates of high blood pressure, diabetes, and obesity are contributing to strokes occurring at younger ages than in the past.

Frequently Asked Questions

Can a stroke be hereditary?

Stroke itself is not directly inherited, but certain risk factors that increase the chance of having a stroke can run in families, including high blood pressure, diabetes, high cholesterol, and certain heart conditions. Having a family history of stroke may mean it is worth discussing your personal risk with a doctor, particularly if multiple close relatives have been affected.

Does stroke always cause permanent disability?

Not always. The outcome of a stroke varies significantly depending on its severity, the part of the brain affected, how quickly treatment was received, and the quality of rehabilitation that follows. Some patients recover fully, particularly after a mild stroke or prompt treatment, while others experience lasting effects of varying degrees. Early treatment and early rehabilitation both play a significant role in maximising recovery.

What is the difference between ischaemic and haemorrhagic stroke?

Ischaemic stroke is caused by a blockage that stops blood from reaching part of the brain, while haemorrhagic stroke is caused by bleeding that damages brain tissue directly and raises pressure inside the skull. Because the two have opposite underlying causes, they require opposite treatment approaches, which is why accurate brain imaging is always carried out before any stroke treatment begins.

Can young people get a stroke?

Yes. While stroke risk increases with age, it is not limited to older adults. Hospital data in Malaysia shows a clear rise in stroke cases among people in their 30s and 40s, often linked to uncontrolled high blood pressure, diabetes, heart conditions, or lifestyle factors. Because stroke is still widely seen as an older person's condition, symptoms in younger patients are sometimes dismissed or mistaken for something else, which can delay urgent treatment.

From Understanding to Action

Understanding what a stroke is, and recognising that it can happen to anyone at any age, is the first step toward acting quickly when it matters most. Knowing the warning signs and how to respond can make a meaningful difference to recovery.

What is a Stroke? A stroke happens when the blood supply to part of the brain is suddenly interrupted or reduced, depriving brain tissue of oxygen and nutrients. Without blood flow, brain cells begin to die within minutes.  The effects of a stroke depend on which part of the brain is affected and how much tissue is damaged. Because different areas of the brain control different functions, a stroke can affect: Movement Speech Vision Memory Swallowing or any combination of these, depending on its location. Stroke is the third leading cause of death in Malaysia, following ischaemic heart disease and pneumonia. It is also a leading cause of long-term disability.  Stroke Emergency? Tumour Markers in Health Screening | What Your Results Mean

Kategori: blog

Common Tumour Markers in Health Screening: What Do They Really Mean? When you go for a health screening, you may come across a group of blood tests called tumour markers. The results can look worrying at first glance, especially when a number appears “high”. But a tumour marker result is rarely the full story on its own. This guide explains what the most common tumour markers are, which cancers they are linked to, and why your results always need to be read together with your doctor.   What Are Tumour Markers? Tumour markers are substances produced by cancer cells, or by the body in response to cancer. They can be detected in blood, body fluids, or tissues. 1. The Main Types of Tests Blood based markers (serum tumour markers) – the most commonly used Imaging tests (radiology based) Tissue tests (biopsy based) 2. Why They Are Used in Health [...]

Scoliosis - Symptoms, Diagnosis and Treatment Options

Kategori: page

Scoliosis is more common than most people realise — and when detected early, many cases can be managed without surgery. What is Scoliosis? Scoliosis is a three-dimensional deformity of the spine characterised by an abnormal lateral curvature — forming a C-shaped or S-shaped curve when viewed from behind. It can affect people of all ages, from children and adolescents during periods of rapid growth, through to adults as a result of age-related spinal degeneration. While mild cases may require only monitoring, more significant curves can affect posture, breathing, and quality of life — and in selected cases, surgical correction may be the most appropriate pathway. Earlier detection and diagnosis allows early conservative treatment such as bracing — which can avoid unnecessary surgery and preserve a higher health-related quality of life. Scoliosis [...]
