This package is valid for six (6) months from the date of purchase; any unused sessions after the validity period will be forfeited.
A doctor’s referral or diagnosis is required before starting the assessment.
Patient must be medically stable before commencement of assessment.
Medication, medical procedures, or additional consultations are not included in this package and will be charged separately if required.
This package is non-transferable and non-refundable once initiated.
